Treatment of Resectable Hepatocellular Carcinoma Including Nonsurgical Treat- ment for Recurrence

Abstract
Introduction
Treatment of hepatocellular carcinoma(HCC) with hepatic resection was a challenge
until the 1970s because of the frequent association of liver dysfunction secondary to
hepatitis virus infection. In .Japan, the proportion of the patients with HCC who are
positive for the hepatitis B surface antigen declined from 40% in late 1970s to 16.8% in
1993, and the hepatitis C virus has played a major role in recent years. Ninety percent
of our HCC patients now are positive for one or more of serum marker of hepatitis B
or C virus. Most of them have associated chronic liver dysfunction. This was the main
obstacle to the successful surgical treatment of hepatocellular carcinoma. However,
safety of hepatic resection has remarkably improved with refinement of surgical
technique, which has afforded opportunity of surgical treatment to increasing number of
patients with HCC.
